| 内容記述 | Hand-over Process 標準化のための要素を明らかにすることを目的とした。AORN のHand-over Process の 引用文献を精読し，要素を整理，分析した。Hand-over Process 標準化の要素は，実施チーム全員のHandover への参加，情報とケアの移行が必要なフェーズでHandover を中断しない環境の整備，ツールの活用であった。 本邦の周術期患者の引き継ぎとHand-over Process との相違性を検討したところ，両者には医療体制の違いが あると考えられた。しかし，効果的なコミュニケーションを促進するために，環境の整備やツールの活用は参 考にすべき要素であった。本邦で患者の引き継ぎの質を向上させるためには，看護師間の引き継ぎに主眼を置 いたHand-over Process の構築が必要である。 |

| 内容記述 | The purpose of this study was to identify the elements for hand-over process standardization. The elements were organized by reviewing the literature cited in AORN's Hand-over Process. The elements of hand-over process standardization include the participation of the entire implementation team in the handover, creating an environment in which handovers are not interrupted during the transition of information and care, and the utilization of tools. Differences were found when we analyzed discrepancies in the medical system between the AORN Hand-over Process and patient handover in Japan. The issues of creating an environment and utilizing tools to facilitate effective communication should also be addressed. Improving the quality of patient handover in Japan requires establishing a hand-over process focusing on the handover between nurses from the medical safety perspective. |
